Introduction
Home-based Chronic Disease Management Service for the Elderly (CDME)
Due to ageing population, over 1 million of population are aged 65 or above in Hong Kong. About 70% of them are suffering from chronic diseases, most commonly diabetes and hypertension. In view of this, HKRC has developed Home-based Chronic Disease Management Service for the Elderly (CDME) since 2014 to serve elders who are unable to go to hospitals or centers and learn about chronic disease self-management. The service help prevent further deterioration of health condition resulted from mismanagement of chronic illness.

Cases are referred by elderly service centres in the community. Elders will be assessed by case manager and registered nurse. Volunteers will visit the elderly regularly and promote health in physical, psychological and social aspects.
Service Content
Service Objectives
- To raise elderly ability in chronic diseases self-management;
- To raise elderly self-efficacy in managing chronic diseases;
- To increase the knowledge of elderly in health-related resources in the community.
Service Targets
Elderly who suffer from diabetes and/or hypertension. Priority will be given to hidden or singleton elderly. Cases have to be referred by elderly services centers.
Service Content
with a view of educating singleton elderly who suffer from hypertension and/or diabetes to know the importance of chronic diseases self-management, the HKRC works closely with elderly service centres in the community. CDME provides "door-to-door" service, staff and health professional assess the health condition of the elderly and develop treatment plans for volunteers to follow afterwards.
Unlike health talks and groups that commonly organized in the community, CDME adopted "case management" approach in helping elderly to manage health. Every elderly would be assessed by professionals and followed up by volunteers and staff. Elders will be visited by same group of trained volunteers regularly within service period of 12-18 months. Strong rapport will be built between volunteers and elderly which make the service more effective.
